Whether you are looking for palliative and respite care, convalescent and rehabilitation time, step down care, chronic fatigue care or end of life care, Burrswood hospital will make every effort to ensure you are in a comfortable, peaceful and safe environment.

For admission to the hospital at Burrswood you will need to complete two forms; both can be downloaded below. Alternatively you can obtain copies by contacting us on 01892 865988 or by emailing admissions@burrswood.org.uk

Please note: Our hospital remains open and fully staffed over Christmas and New Year 2011.  We are ready to take admissions, transfers from other hospitals and those requiring respite care in a skilled and safe environment where Christmas is celebrated to the full and a programme of events runs from 23 to 29 December.

Patient's Admission Form 
Currently admissions forms can be downloaded, printed and completed by hand and returned to our admissions team by post, fax or attached to an email as a scanned image.  On receipt of your form our admissions team will contact you to discuss your requirements.  In the near future we will be able to offer the option of completing and submitting admissions forms electronically. Doctor's Form
The Doctor's Form, along with the covering letter, should be passed to your GP for him/her to complete and return direct to us. Your doctor may charge for this medical referral to be completed and this charge will be your responsibility. If you are requiring admission quickly, you may prefer to fax the forms back to us on 01892 862597. As soon as we have received the two completed forms the application will be considered by our Admissions Team, who meet most weekdays.  We shall then contact you as soon as possible to discuss your admission with you.

Access to Care Form
We seek to ensure that all those in need of Burrswood’s care are enabled to receive it, regardless of ability to pay.  As a charity, we subsidise the true bed cost, and additionally operate a fund to which individuals who suffer genuine financial hardship may apply for assistance. We welcome adults regardless of age, religion or ethnicity and offer financial support with the cost of care as wisely and fairly as we can. Fast Track Admissions

For same-day admissions, the referring doctor needs to call 01892 863637 before 2pm and ask to speak to the Duty Physician. Every effort will be made to provide a bed in our hospital that day, when the need for urgent admission is absolute and there is a bed available.
